The Licensed Clinical Social Worker will provide psychosocial and mental health intake assessments, maintaining close liaison with other care providers involved in patient treatment. Responsibilities also include preparing necessary documentation like initial assessments, treatment plans, and daily notes in a timely manner, and informing patients about the PCMH program.
Candidates must possess a Master’s Degree from an accredited School of Social Work and a minimum of two years of clinical experience, along with an active NYS Licensed Clinical Social Worker credential. Successful performance requires strong language, basic mathematical, and reasoning abilities, along with proficiency in Microsoft Office and EMR software.

Education and/or Experience
