The Speech-Language Pathologist will perform evaluations, determine diagnoses, develop and execute plans of care, and conduct therapy procedures to restore or maintain speech, language, motor, and swallowing abilities. This role also involves updating care plans, coordinating with Home Health Aides, participating in team meetings, and documenting skilled intervention outcomes.
Requirements summary
Candidates must possess a Master's Degree in Speech Language Pathology and hold required certifications including CCC-SLP from ASHA, Virginia licensure, and CPR/BLS certification. One year of Speech Language Pathology experience is preferred, along with demonstrated knowledge of growth and development principles across the lifespan.
Gloucester, Virginia Overview A staff Speech-Language Pathologist will follow appropriate practices to perform evaluations, timely re-evaluations and OASIS assessments, determine SLP diagnoses, develop a plan of care, and perform SLP procedures to aid in restoring/maintaining speech, language, motor, and swallowing abilities.
This position must also be able to follow and adhere to the practices described in the Department Policy and Procedure Manual.
The Therapist is required to be flexible with daily scheduling and may be required to work various patient care settings .
May need to assist in coordination of Home Health Aides, participation in interdisciplinary case management, completing quality chart reviews, participating in meetings as required by facility/department, and engagement in other initiatives to improve quality outcome.
What you will do Performs evaluations, determines SLP diagnoses, develops plan of care, and performs speech therapy procedures to aid in restoring/maintaining speech, language, motor, and swallowing abilities.
Evaluates and assesses the patient timely, based on principles outlined by practice guidelines, evidence based guidelines and regulatory standards (state & organizational policies, requirements of accrediting bodies, etc.
Provides reassessments including OASIS assessments at appropriate time points to determine the level of functioning.
Updates and revises the plan of care in consultation with the physician and interdisciplinary team members.
Provides therapeutic treatments and evaluation of equipment needs as applicable.
Prepares clinical and progress notes that support skilled intervention and evaluates assesses outcomes of care.
Plans for discharge of patient.
Builds trusting collaborative relationships with a multi-disciplinary team to support patient care needs.
Actively participates in multi-disciplinary team meetings, in-services, and case conferences.
Carries out treatment plan effectively and modifies to maximize therapeutic benefit and meet discharge criteria and return to optimal level of care.
Utilizes testing results and develops updated plan of care based on principles outlined in appropriate evidence based guidelines.
Coordinates with Home Health Aides in providing services including completion of required supervisory visit documentation.
Participates in training and support of all team members, volunteers and students.
Qualifications Education Masters Degree, Speech Language Pathology (Required) Experience 1 year Speech Language Pathology (Preferred) Skills and Abilities Demonstrates knowledge of the principles of growth and development throughout the life span, be able to assess patients' needs and status by analyzing evaluated data, and be able to interpret information to identify age-specific needs and provide appropriate care accordingly per patient care setting.
