About the role

Primary Care Advanced Practice Provider (PA-C/ARNP) – Compass Clinic

Position Overview

We are seeking a Primary Care Advanced Practice Provider (PA-C/ARNP) who thrives in a value-based, patient-centered care environment. The ideal candidate will work closely with a dedicated interdisciplinary team— including a Physician, nurse care manager, and clinical pharmacist—to deliver high-touch, coordinated care to a defined patient panel.

This is not a typical fee-for-service role. Our providers focus on long-term outcomes rather than visit volume. In this role, you will collaborate with other physicians, nurses, therapists, and pharmacists to deliver comprehensive care to patients enrolled in value-based care programs, such as Medicare Shared Savings Programs (MSSP), and MedAdvantage. Your contributions will be pivotal in reducing the total cost of care, improving quality metrics, and ensuring equitable access.

Responsibilities

  • Serve as the primary point of accountability for a panel of patients, focusing on preventive, chronic, and acute care management.
  • Collaborate daily with a multidisciplinary team, including Physicians, APP, RN, and clinical pharmacist to optimize patient outcomes.
  • Utilize population health tools and data analytics to proactively identify and close care gaps, reduce hospitalizations, and manage high-risk patients.
  • Participate in regular case conferences, team huddles, and quality improvement initiatives.
  • Minimize unnecessary utilization through evidence-based, cost-conscious decision-making.
  • Leverage technology (e.g., EHRs, telehealth, remote monitoring) to extend care beyond traditional visits.
  • Participate in quality reporting programs and value-based performance measurement.

Qualifications

  • PA-C or ARNP/FNP/AGNP from an accredited school
  • Active and unrestricted medical license in WA
  • Passion for team-based care and experience in a collaborative clinical setting preferred
  • Experience in value-based care, risk-based contracts, or population health is a strong plus
  • Strong communication and leadership skills
  • Comfortable working with EHR systems and data analytics tools
  • Minimum of 2 years of clinical experience, preferably in primary care or a value-based care setting
  • Preferred Experience
  • Prior work in risk-based or capitated environments.
  • Familiarity with HEDIS measures, Medicare Advantage plans, and/or Accountable Care Organizations (ACO).
  • Experience working with medically complex and socially vulnerable populations.

Compensation

Base Salary of $135,000 - $150,000 + Strategic Incentive

Requirements

Candidates must possess a PA-C or ARNP/FNP/AGNP certification from an accredited school along with an active and unrestricted medical license in Washington State. A minimum of two years of clinical experience, preferably in primary care or value-based settings, is required, alongside strong communication and comfort with technology.
