The Community Nurse will conduct health assessments, ongoing monitoring, and coordinate medical appointments and follow-ups for individuals supported by the organization. This role involves partnering with the Medical Services Team and collaborating with interdisciplinary teams to ensure high-quality, person-centered care and promote independence.
Candidates must possess an active, valid New York State Registered Nurse (NYS RN) license and a minimum of two years of nursing experience. Strong organizational and coordination skills, along with a passion for community-based, person-centered support, are essential, with OPWDD experience being highly advantageous.
