Physical Therapists evaluate and implement individualized treatment plans to improve mobility, restore function, maintain safety, and reduce pain for participants, collaborating with the Interdisciplinary Team to provide all-inclusive care for aging safely at home. Specific duties include performing assessments, providing treatment sessions, implementing care plans, ordering Durable Medical Equipment, and documenting all care provided according to department standards.
Candidates must possess a Physical Therapy degree (Bachelor, Master, or Doctoral) and hold an active State of Michigan licensure. A minimum of one year of experience working with a frail or elderly population is required, along with the physical ability to instruct exercises, provide lift assistance, and manage mobility devices.
