The primary purpose of this role is to plan, organize, and implement social services for PACE Organization participants and their families, which includes assessment, treatment, teaching, and counseling.
Candidates must possess a Master's degree in Social Work and have two years of experience, including one year working with frail or elderly populations.
Must possess a comprehensive knowledge of social work as normally obtained through a Master's degree in Social Work. Two (2) years of experience, including one (1) year working with the frail or elderly population. Current license in social work within the state of PACE Organization preferred but not required Experience working on a multidisciplinary team in a hospital, nursing home or community-based setting is preferred. Experience and thorough knowledge of social service principles and practices. Knowledge of financing mechanisms such as Medicare, Medicaid and other payment systems. Valid driver's license and ability to provide a reliable means of transportation required. Position may require 10% travel within the Centers geographic region. Position requires ability to engage in frequent kneeling, crouching, reaching, lifting, sitting, walking, bending and lifting. Must be able to push, pull, move and or lift a minimum of 20 pounds if applicable.
