The Palliative Care Nurse Practitioner provides comprehensive diagnosis, treatment, and management of patients with serious or progressive illness. They conduct patient histories, physical examinations, and develop individualized plans of care in collaboration with an interdisciplinary team.
Candidates must have a Master’s Degree in Nursing and an active Georgia license with board certification as a Nurse Practitioner. A minimum of three years of nursing experience, particularly in pain and symptom management for serious illnesses, is required.
Hospice Savannah Inc. - SAVANNAH, GA 31406Job Summary The Palliative Care Nurse Practitioner (NP), practicing under the clinical supervision of a licensed physician, provides comprehensive diagnosis, treatment, and management of patients with serious or progressive illness. Working within established nurse practitioner parameters, the NP delivers hospice and palliative care services focused on symptom management and goals-of-care alignment. As a primary care provider and hospice/palliative care specialist, the NP conducts patient histories, physical examinations, and assessments, and develops individualized plans of care addressing physical, psychological, emotional, social, and spiritual needs. Care is delivered collaboratively with the interdisciplinary team and in coordination with covering physicians as appropriate. The NP may provide services across multiple care settings, including hospitals, clinics, private homes, nursing facilities, and assisted living communities, while consistently demonstrating Hospice Savannah’s competencies, values, and service excellence standards. The position is accountable to and evaluated by the Steward Center Medical Director. QualificationsMinimum Qualifications Master’s Degree in Nursing with successful completion of an accredited Nurse Practitioner program. Active Georgia license and current board certification as an Adult, Family, or Gerontology Nurse Practitioner. Minimum of three years of nursing experience, including demonstrated experience in pain and symptom management for patients with serious or life‑limiting illness. Ability to work effectively as part of an interdisciplinary team to support patients and families. Strong verbal and written communication skills with the ability to interact professionally with physicians, staff, patients, families, and the public. Basic computer proficiency with ability to navigate EMRs. Physical Demands and Work Environment Position requires moderate to intense visual concentration, reading and typing on a computer; May be required to lift up to 20 pounds. Position Type and Expected Hours of Work Position is 40 hours per week; Monday-Friday 8a to 5p. Travel Must be able to travel as required Position requires some travel between Hospice Savannah locations requiring a personal vehicle and valid driver’s license
