This is an on-site position located at Newton-Wellesley Hospital.

  • Responsibilities: The position of Hospital-Based Hospice Nurse Liaison (HBHNL) is to provide information about hospice services to patients, families, and hospital-based staff, to conduct clinical assessments, coordinate hospital discharge plans, and maintain professional relationships within hospitals, with a focus on the assigned hospital.

Excellent problem-solving skills Competency with accepted standards of hospice nursing practice.
