The hospice nurse practitioner will join our expanding interdisciplinary team providing care for hospice patients in the community setting.

• Responsibilities: Complete face-to-face visits for initial and ongoing hospice eligibility, assessing disease-related changes, declines in functional and cognitive status, and symptom burden, including effectiveness of current interventions Communicate significant visit findings (including unmanaged symptoms and eligibility concerns) to the designated hospice physician, in a timely fashion Engage with, and work alongside, other members of the hospice interdisciplinary team Provide symptom management recommendations for current hospice patients, as necessary Prescribe medications for current hospice patients, as necessary

• Experience: Master’s degree from an accredited program and a current nurse practitioner license valid in the state of Massachusetts 2 years of nursing experience with at least 1 year of experience in hospice, palliative care, or oncology Familiarity with Medicare Part A hospice regulations and eligibility requirements A professional and caring demeanor Ability to take initiative, work independently, and manage time well Outstanding communication skills and willingness to converse with other members of the hospice interdisciplinary team, medical staff in the community, and patients and their families Comfort working with culturally diverse patients/families Able to document in an electronic medical record, after adequate training
