Travel is required for this position. Care is provided in patient’s private homes and residential facilities throughout Greater Boston and parts of Metrowest.

  • Responsibilities: The social worker is an integral part of an interdisciplinary hospice team responsible for providing comprehensive psychosocial care to patients and families at the end of life. Makes home visits, facilitates referrals and admissions to services and provides counseling support during the bereavement period.

Requirements
