The physical therapist is responsible for assessing and evaluating patients' therapeutic and rehabilitative needs and developing a total plan of care. They also direct physical therapy treatment and provide education to patients and caregivers.
Requirements summary
A Bachelor’s Degree in Physical Therapy is required, with a Master’s Degree preferred. Candidates should have at least 2 years of experience and hold a PT License and CPR certification as required by the state.
The physical therapist is responsible for proving physical therapy services and for adherence to all conditions outlined in the personnel services agreement.
ESSENTIAL RESPONSIBILITIES: Assessing and evaluating therapeutic, rehabilitative, functional status and participation in the development of the total plan of care.
Evaluates home environment.
Assesses for muscle strength, mobility, gait, ROM – potential for rehab.
Directing physical therapy treatment.
Initiates physical therapy program and instructs other personnel and/or family /caregiver members in certain phases of physical therapy with which they may work with a patient, as well as instructing them as to the goals of the physical therapy program for the patient by participating in case conferences.
May train patient in the use of prosthetic device.
Participates in discharge planning for patient and in peer consultation process.
Provides physical therapy consultation to home families/caregivers when indicated and provides Inservice education programs for nursing organization personnel as needed.
When therapy is only skilled service, instructs, supervises and evaluates home health aide care every two weeks.
EDUCATION AND WORK EXPERIENCE: Required Degree: Bachelor’s Degree in Physical Therapy approved by an accredited organization.
Preferred Degree: Master’s Degree in Physical Therapy approved by an accredited organization.
Certificate(s): PT License and CPR certification, if required by state Experience: 2+ Years KNOWLEDGE, SKILLS, AND ABILITY: Ability to show compassion and understanding.
Knowledge in HIPAA laws and confidentiality policies.
Ability to create and implement treatment plans based on patients’ specific condition and needs.
Ability to evaluate, conduct and development customized treatment plans based on findings.
Strong observation, time management, detail orientation, communication and interpersonal skills.
PHYSICAL REQUIREMENTS: Ability to stand, sit, talk, reach, stoop, kneel, and use hands and fingers to operate a computer, telephone, and keyboard.
Ability to lift, carry, push, and/or pull light to moderate amounts of weight up to 100 lbs.
