The Nurse Practitioner will provide comprehensive primary care services, including diagnosing, treating, and managing patient health within a collaborative care team. They are responsible for documenting clinical progress, performing physical exams, and educating patients on disease management and wellness.
Candidates must hold a Master’s Degree in Science (APN) and possess a valid Louisiana Registered Nurse license and Advanced Practice Nursing Certificate. At least one year of experience is required, with board certification and relevant clinical certifications such as BLS/ACLS.
Practitioner is an Advanced Practice Registered Nurse, skilled and able to provide the full range of core primary care services including evaluating, diagnosing, performing history and physical exam, consulting, and providing non-surgical treatment to patients presenting with primary care or general medical problems within the scope of their collaborative agreement. Assures clinical guidelines are followed when providing patient care. Keeps up with changing guidelines regarding general patient care and patient-specific clinical conditions. Medical Care: Serves as the medical provider on CrescentCare Care Teams responsible for the overall care of patients as defined within their agreement with their collaborating physician(s). Incorporates wellness, quality management, and prevention into all visits Works with CrescentCare’s existing programs to target specific health needs in our community, including but not limited to: HIV elimination, HCV eradication, Women’s Health and contraception education, Gender Care, treatment of substance use and opioid use disorder, and general primary care Maintains clinical competency, as demonstrated in providing and directing patient care. Core Provider Responsibilities: Obtain and document a health history; Perform and document complete, system-focused, or symptom-specific physical examination; Assess the need for and perform additional screening and diagnostic testing, based on initial assessment findings; Provide specialty specific consultation services upon request and within specialty scope of practice; Prioritize data collection; Perform clinic visits on assigned patient population; Document clinical progress notes, plan of care, evaluation and discharge summary; Manage diagnostic tests through ordering and interpretation; Formulate differential diagnoses by priority; Prescribe appropriate pharmacologic and non-pharmacologic treatment modalities; Utilize evidence-based, approved practice protocols in planning and implementing care; Initiate appropriate referrals and consultations; Obtains proper informed consent as a prerequisite to any procedure requiring informed consent. This information must include the signature of the responsible practitioner. Teamwork and Health Promotion: Assist in educational programs for staff and participates in team meetings. Provide education to patient and patient support systems in areas such as disease management, safer sex, adherence, etc. Communicate with medical staff concerning patient care issues and function as a part of primary care team using evidence-based practices. Promote teamwork, good interpersonal relations and communication among staff at the healthcare setting. Participate in Health Fairs, employee and volunteer health programs, immunization campaigns, and home visits as appropriate Facilitate the patient’s transition between and within health care settings, such as admitting, transferring, and discharging patients.
