About the role

The therapist will provide integrated rehabilitation, treatment, and crisis intervention services to individuals with serious mental illness within their homes and the community. They are also responsible for maintaining accurate electronic clinical records and coordinating services across multiple health systems.

As a Therapist, you will work within our Program of Assertive Community Treatment (PACT) team located in Cherry Hill, Camden County NJ. PACT provides comprehensive, integrated rehabilitation, treatment and support services to individuals with serious and persistent mental illness, who have had repeated hospitalizations and are at serious risk for psychiatric hospitalization.

Schedule: Full-time; Monday-Friday; Flexible schedules; Community-Based

Salary: $50,000-$55,000/year,

Responsibilities

  • Work in-home and in the community;
  • Work collaboratively with the participating providers;
  • Provide crisis intervention;
  • Responsible for maintaining a consumer clinical record (electronically) that accurately reflects all services provided on a timely basis and meets Agency and funding source standards;
  • To monitor and oversee service linkage in the mental health and non-mental health, health system; coordinating and integrating services from multiple providers;
  • To monitor and oversee the provision of community-based services for consumers enrolled in the PACT program;
  • To provide clinical insight and treatment working with the PACT team.

Qualifications

  • Master's Degree in a behavioral health science or counseling specialty from an accredited institution and have two years of post-bachelor’s experience working with individuals with severe and persistent mental illness;
  • Must be willing to work in-home and in-community based settings;
  • Valid Driver's License required

Requirements
