Provide high-quality primary care services to seniors and families within a value-based care model. Manage chronic diseases and preventative care while maintaining excellent patient relationships and accurate documentation.
Must hold an active and unrestrictive Texas APRN license and be eligible for a DEA license. Requires 2-3 years of experience in primary care or value-based care settings with Medicare and Medicaid populations.
Have at least 2-3 years' experience in the primary care/value-based care setting, working with Medicare Advantage, Medicare, and Medicaid patient populations Must have an active and unrestrictive Texas APRN license Must have DEA license or eligible to obtain it within 2 months of date of hire Strong understanding of chronic disease management, preventative care, and quality metrics in a value-based care model Excellent documentation and patient relationship-building skills Proficient with EMR systems (eCW a plus!)
