The LPN Program Coordinator will manage patient care for the GRACE study, including scheduling appointments, coordinating with outside agencies, and maintaining care plans. They will also conduct Medicare Annual Wellness Visits, perform clinical procedures, and ensure accurate documentation in the EHR.
Candidates must hold a current, unrestricted LPN license and have graduated from an accredited practical nursing program. Experience in outpatient primary care or internal medicine, along with familiarity with CMS preventive care guidelines, is required.

The SPIRE study is seeking a full time LPN Program Coordinator for patients who are enrolled in the GRACE arm of study. The LPN would have the following
responsibilities: · Check patients in/out in the EMR System
· Schedule appointments
· Coordinate patient care with outside agencies such as mobile imaging, blood draw, VNAs, etc.
· Maintain GRACE patient by conducting items such as monthly Chronic Care Management, TCM calls, scheduling TCM appointments and ER Discharge appointments.
· Monitor GRACE patient implemented care plans and coordinate care such as referrals, hospitalizations, patient education, etc.
· Call patients with results
· Manage DME orders, prior authorizations, home care orders, and clinical paperwork (FMLA, Medical Transportation, Assisted Living, etc.)
· Triage patient medical complaints/concerns.
