The Advanced Practice Provider collaborates with center leadership and medical directors to deliver primary and urgent care services to nursing center residents. They are responsible for achieving clinical goals and improving health outcomes through compassionate, high-quality medical care.
Candidates must be a licensed Nurse Practitioner or Physician Assistant with at least one year of provider experience. A Master's degree in Nursing or an ARC-PA accredited program degree is required, along with relevant state licensure and DEA certification.

We are seeking a provider for a Part Time (2-3 days per week) position split between two centers located in Frederick, MD and Walkersville, MD: Autumn Lake Healthcare at Ballenger Creek and Autumn Lake Healthcare at Glade Valley. This position can be filled by a nurse practitioner or physician assistant. 1 year or more of provider experience is required.
