The Hospice Home Team Weekend RN conducts comprehensive patient assessments and administers treatments to ensure comfort and symptom control during weekend shifts. They also collaborate with interdisciplinary teams to update care plans and provide emotional support to patients and their families.
Candidates must hold a current Registered Nurse (RN) license and have at least two years of clinical nursing experience in hospice, palliative, or home care. A Basic Life Support (BLS) certification is required, and the ability to obtain CHPN certification within three years is expected.
The Hospice Home Team Weekend RN plays a critical role in delivering compassionate, patient-centered care to individuals facing life-limiting illnesses during weekend shifts.
Hours: Every Other Saturday/Sunday from 8am-8pm
Territory will include Sheboygan and Manitowoc Counties, with the potential of Ozaukee County as well.
This position ensures continuity of care by managing symptom control, providing emotional support to patients and their families, and coordinating with interdisciplinary teams to uphold the highest standards of hospice care. The RN will assess patient needs, implement care plans, and respond promptly to changes in patient conditions to enhance comfort and quality of life. This role requires a strong commitment to holistic care, respecting patient dignity and preferences while navigating complex clinical situations. Ultimately, the Hospice Home Team Weekend RN contributes significantly to the mission of providing empathetic, expert care during some of the most vulnerable moments in patients' lives.
