Perform initial and ongoing patient assessments while developing and implementing tailored treatment plans in a home health setting. Collaborate with interdisciplinary teams and supervise physical therapy assistants to ensure high-quality, patient-centered care.
Candidates must hold a current physical therapy license and a valid BLS certification. A minimum of one year of physical therapy experience is required, along with a valid driver's license and reliable transportation.
