As a Primary Care Advanced Practitioner, you will provide comprehensive primary care services, manage chronic conditions, and promote wellness through education. You will work collaboratively within a care team to ensure continuity and quality of care.
Candidates must be a Certified Nurse Practitioner or Physician Assistant with at least 1 year of experience in primary care, preferably with Medicare patients. An active state license, DEA registration, and a passion for patient-centered medicine are also required.
