Gaudenzia's Mission is to promote long-term recovery by providing comprehensive treatment and support to people with substance use disorder, co-occurring mental health disorders, and related conditions, regardless of individual circumstances.
This individual's responsibility will be to design, develop, implement, supervise, and provide specialized training and services to clients in the program. In addition, this person may also handle an independent case load or assist other personnel in the counseling of clients to aid them in achieving adjustments to specific problems or situations of a difficult or complex nature.

Your responsibilities in this role may include
Responsible for conducting intake interview and orientation to care coordination, with client prior to discharge from residential level of care
Responsible for liaison with clinical staff for review and verification of recommended services, referral to clinically recommended level of care, and collateral contact information
Completion of Recovery Plan in coordination with client and primary clinical counselor
Responsible for identifying health care and recovery support needs with client and initiating referrals
Responsible for assisting clients in getting to the next level of care (if applicable) by removing barriers, providing reminders, and conducting outreach when appropriate
Responsible for developing and maintaining relationships with services and programs. These linkages will include but are not limited to treatment, health care, employment, legal, housing, child welfare, social services, peer-support providers, etc.

Additional duties to support the existing team:
Assisting residents in education around program offerings/supports and ways to access
Reducing barriers to care by addressing logistical, financial, cultural, and systemic challenges
Ensuring client medical, psychiatric, and outpatient needs are being met and referrals responded to within a timely manner
Ensuring education of day to day symptom management, medication management, and comorbid conditions
Coordinating timely access to appropriate RVWV and community providers based on clinical recommendations and resident disposition throughout treatment
Ensuring resident community appointments are maintained
Maintaining engagement during transitions
Linking patients to recovery supports and community resources
Conduct post-discharge follow-up
Assist in reducing readmissions by providing interventions based upon post discharge follow up responses from residents Documentation, Quality & Compliance
Maintain accurate EHR documentation
Track access, engagement, and outcomes
Comply with HIPAA and 42 CFR Part 2
Participate in performance improvement and satisfaction initiatives
Skills, Knowledge, and Abilities
Specialized training and experience in working with specific population
Knowledge of the basic principles, practices, and procedures involved in an alcohol an, drug program
Knowledge of drug and alcohol abusing subculture
Knowledge of the physical and emotional characteristics and manifestations of alcohol and drug dependency
Knowledge of individual and group behavior and the effective ways of working with them
Knowledge of the continuum of care as it relates to drug and alcohol treatment
Knowledge of confidentiality and all other applicable federal and state regulations in reference to specialty area, and ability to ensure program compliance
Ability to perform needs assessment and develop appropriate programming
Ability to work effectively with people and aid them in adjusting to specific problems
Ability to establish and maintain effective working relationship with clients, and other members of staff, outside agencies and institutions, and the general public
Ability to design, implement and train others pertaining to the specialty area
Ability to communicate effectively and clearly, both verbally and in writing
Ability to present effective seminars, workshops, and presentations to clients, staff and the community
Ability to work independently and exercise independent judgment
