We are currently hiring a Community Health Worker based in Milford MA. The Community Health Worker (CHW) is a key member of the health care team, providing comprehensive and coordinated interventions to address the social determinants of health (SDOH) as part of a patient-centered medical home. The CHW facilitates access to care and services by providing education and advocacy to assist patients in navigating the health care and social service systems.

Conduct psycho-social assessments with patients referred for Community Health Worker interventions in accordance with Health Center policies, procedures and protocols.
Provide health system education and advocacy to assist patients in navigating the health care system.
Provide basic education, screening, brief intervention and/or facilitate referrals to other health center or community-based resources in accordance with applicable health center protocols.
