• The Community Health Worker (CHW) is a key member of the health care team, providing comprehensive and coordinated interventions to address the social determinants of health (SDOH) as part of a patient-centered medical home.
• The CHW facilitates access to care and services by providing education and advocacy to assist patients in navigating the health care and social service systems.

Essential Functions

Conduct psycho-social assessments with patients referred for Community Health Worker interventions in accordance with Health Center policies, procedures and protocols. Provide health system education and advocacy to assist patients in navigating the health care system. Provide basic education, screening, brief intervention and/or facilitate referrals to other health center or community-based resources in accordance with applicable health center protocols. Job Requirements

Basic knowledge of Microsoft Office and ability to learn the health center’s Electronic Medical Record and other applications. Successful completion of the CHW core competency training within 2 years of employment. BLS Certification, can be obtained through the Health Center
