The Speech Language Pathologist will conduct home-based evaluations and develop personalized treatment plans to address speech and language functional needs. They will also provide therapeutic interventions, educate caregivers, and maintain accurate clinical documentation within the electronic medical record system.
Candidates must hold a Master's or doctoral degree in speech-language pathology and possess a valid state license. A minimum of one year of clinical experience in speech-language pathology is required.

Conduct evaluations and assess home environments to identify speech/language function needs. Develop and implement plans of care to address functional speech defects. Administer specialized therapeutic techniques and recommend communication aids as needed. Instruct patients, families, and caregivers on treatment regimes and collaborate with other healthcare personnel. Maintain accurate clinical notes and participate in discharge planning using the company's electronic medical record system. Attend in-services, case conferences, and meetings to ensure coordinated patient care. Maintain clinical competency in speech-language pathology practices. May supervise home health aides related to the speech-language therapy plan of care.
Master’s or doctoral degree in speech-language pathology (SLP). Licensed as a speech-language pathologist by the state or fulfilling requirements if the state does not license SLPs. Minimum of one (1) year's clinical experience in Speech-Language Pathology. Strong verbal and written communication and organizational skills.
