The Care Manager will conduct comprehensive needs assessments and develop individualized care plans for consumers. They will also facilitate the coordination and monitoring of home care services through regular contact and home visits.
Candidates should have at least one year of experience in the human services field. A bachelor's degree in Social Work or a related field is highly preferred.
