The Clinician Case Manager provides direct services including evaluations, treatment plans, and family therapies using the Functional Family Therapy model. They are responsible for community-based outreach, case management, and maintaining collaborative relationships with families and professional team members.
Must have the ability to maintain the utmost confidentiality of both agency and client matters. Must understand and be able to display the clinical knowledge and expertise necessary to perform crisis intervention. Must be able to display knowledge and expertise in clinical theory and practice as it relates to the populations served by Wellmore. Must be capable of working as a part of a team and be able to effectively communicate with both families and professionals (in writing and verbally). Working knowledge of the greater Waterbury area is a must! Ability to read, write, translate and interpret Spanish/English. EDUCATION & EXPERIENCE: A Master’s degree in Marriage and family therapy, social work or related human services field. At least one (1) year clinical internship workign with children and/or adolescents or families required. An active license with the CT Dept. of Public Health as a Marriage and Family Therapist, Professional Counselor, Licensed Clinical Social Worker or Doctorate Psychologist preferred (i.e., LMSW, LCSW, LMFT, LMFT-A, LPC, or LPC-A). Two (2) years post graduate clinical experience working with children, adolescents or families is preferred. Experience in family therapies as well as family systems theory and practice is preferred. LICENSURE/CERTIFICATION/REGISTRATION: Valid driver’s license, reliable transportation and valid auto insurance; CPR and first-aid.
Candidates must hold a Master’s degree in Marriage and Family Therapy, Social Work, or a related field and possess at least one year of clinical internship experience. A valid driver’s license and the ability to communicate in both Spanish and English are required.
