The Clinical Pharmacist compounds and dispenses prescribed drugs according to physician instructions and consults with medical staff regarding drug interactions and patient medical history. They also respond to patient inquiries about medications and ensure compliance with pharmacy data regulations.
Candidates must have a Doctor of Pharmacy or a Bachelor's degree in pharmacy, with a pharmacy residency preferred. A valid Registered Pharmacist license from the South Carolina Board of Pharmacy and current BLS certification are required.
Summary The Ambulatory Clinical Pharmacist provides direct patient-centered clinical services, including Medicare Annual Wellness Visits (AWVs), comprehensive medication reviews, chronic disease management, and preventive care services. This role works collaboratively with physicians, advanced practice providers, and care teams to identify medication-related issues, support quality outcomes, and optimize patient safety and health. Minimum Training and Education: Doctor of Pharmacy or a Bachelor's degree in pharmacy. Completion of a pharmacy residency preferred.
