The Assessor provides substance use level of care assessments based on ASAM criteria and maintains accurate patient records. They also coordinate continuum of care activities and ensure compliance with regulatory standards and funding requirements.
Candidates must possess a minimum of a Master's level degree in a related field. Experience providing level of care assessments for substance use and co-occurring populations is required.
The primary responsibilities of the Assessor are: Provide substance use Level of Care Assessments via assessing the patient's needs and determine appropriate level of care based on placement criteria established by the American Society of Addiction Medicine (ASAM) Maintain patient chart via electronic monitoring records with accurate and complete documentation Abide by regulatory compliance, local, state and federal laws including HIPPA and 42 CFR Coordinate continuum of care activities for individuals seeking assessment, provide access to available ancillary services, and ensure optimum compliance with external agencies and referral source requirements Facilitate appropriate funding arrangements
Experience providing level of care assessments to SUD and co-occurring population required and/or experience providing screening and assessments per ASAM guidelines.
