Pediatric Physical Therapist (PT) | Full-Time | Pediatric Outpatient & PPEC | Jacksonville, FL (Southpoint) At Great Strides Rehabilitation, an H2 Health Company, we believe great care begins with great clinicians.
As a Pediatric Physical Therapist (PT), you’ll provide compassionate, evidence-based care across our pediatric outpatient and PPEC (Prescribed Pediatric Extended Care) programs, helping children improve mobility, independence, and participation in daily life while advancing your career in a collaborative, clinician-led environment.
Our clinicians primarily serve children with autism spectrum disorder, developmental delays, global developmental delay, neurological conditions, genetic disorders, and sensory-motor challenges.
We focus on helping children build strength, coordination, balance, mobility, and independence through evidence-based, child-centered care that supports developmental milestones.
Your Role: Deliver personalized, evidence-based pediatric physical therapy to children with developmental and neurological diagnoses Develop treatment plans focused on gross motor skills, balance, coordination, mobility, and functional independence Use play-based and child-led interventions to support engagement and progress Document care accurately in Raintree EMR Partner closely with families and caregivers to promote carryover at home Collaborate with occupational therapists, speech-language pathologists, and interdisciplinary team members Contribute to a culture of clinical learning and peer support Provide skilled therapy services within both pediatric outpatient and PPEC settings Collaborate with interdisciplinary teams supporting medically complex and developmentally delayed children Deliver family-centered care focused on functional mobility, positioning, transfers, gait, balance, and developmental progression Degree from a CAPTE-accredited Physical Therapy program Active state Physical Therapy license or eligibility for licensure Strong communication, interpersonal, and clinical reasoning skills Passion for working with children and delivering family-centered care Commitment to compassionate, patient-centered therapy Why Great Strides Rehabilitation?
