The Licensed Practical Nurse will conduct patient assessments, administer medications, and provide direct clinical support to healthcare providers. They will also document patient records, educate patients on health management, and collaborate with multidisciplinary teams to ensure high-quality care.
Candidates must hold a valid Licensed Practical Nurse (LPN) license in Tennessee. Previous experience in a healthcare setting, such as an FQHC or primary care clinic, is preferred along with proficiency in EMR systems.

We have two (2) open positions, one (1) is for our Westmoreland office and one (1) is for our Gallatin office.
We are seeking a skilled and compassionate Licensed Practical Nurse (LPN) to join our dedicated team at our Federally Qualified Health Center (FQHC). As an LPN, you will play a vital role in providing comprehensive healthcare services to our diverse patient population. Your expertise and commitment will contribute to our mission of delivering high-quality, accessible, and patient-centered care to individuals in need. Although it is not typical, Off-Site Work could, at times, be required of this position. This off-site work will typically include, but is not limited to, performing nursing duties (i.e. Vital Signs) at Health Fairs or other HOPE Sponsored Events and attending off-site meetings, trainings or conferences.
