The Licensed Practical Nurse provides direct patient care, administers medications, and monitors resident health status under the supervision of an RN or physician. They also support Certified Nursing Assistants and maintain accurate clinical documentation to ensure high-quality care.
What You'll Do Provide direct patient care under the supervision of a Registered Nurse (RN) or physician in accordance with care plans and facility policies.
Administer medications and treatments as prescribed while monitoring residents for reactions or changes in condition.
Monitor residents’ vital signs, health status, and overall well-being, reporting changes to the RN or physician.
Assist with the development and implementation of patient care plans.
Support and supervise Certified Nursing Assistants (CNAs) in providing daily care to residents.
Maintain accurate and timely documentation of patient care, treatments, and observations.
Ensure residents receive assistance with daily living activities and maintain comfort and dignity.
Maintain a clean, safe, and supportive environment while following infection control and safety protocols.
Communicate effectively with residents, families, and the healthcare team to support quality patient care.
Active New Jersey Licensed Practical Nurse (LPN) license CPR/BLS Certification Fully COVID-19 vaccinated with booster (documentation required) Flu vaccination with documentation Valid state-issued ID or driver’s license Ability to work scheduled shifts (7:00am–3:00pm, 3:00pm–11:00pm, or 11:00pm–7:00am) Strong clinical and patient care skills Excellent communication and interpersonal skills Compassionate, dependable, and patient-focused attitude Ability to pass background screening and required employment verification
