The Nurse Practitioner will manage the acute, subacute, and primary care needs of patients by rounding at assigned facilities. They will collaborate with the OnSite team and nursing home staff to provide interventions and patient education.
Candidates must hold a Master of Science in Nursing with a relevant NP certification and an active South Carolina license. At least one year of experience and strong assessment and documentation skills are required.
