The advanced practitioner provides comprehensive primary care to individuals and families within a team-based Patient-Centered Medical Home model. Responsibilities include examining, diagnosing, and treating patients, as well as participating in quality assurance and care coordination initiatives.
Candidates must hold a current Massachusetts NP or PA license and possess certification in their respective specialty. A minimum of two years of experience in an ambulatory care setting is required, with community health center experience preferred.
Description Position Overview Under the general direction of the Chief Medical Officer (CMO) and the direct supervision of the Medical Director, the advanced practitioner will provide continuing and comprehensive health care for the individual and family across all ages and genders within the context of Patient-Centered Medical Home (PCMH) policies and procedures. The clinician will exercise individual initiative and independent action to ensure high-quality and comprehensive primary care. The two central tenets of PCMH lie in team-based care and care coordination. The advanced practitioner is expected to be an integral part of the Primary Care Team (PCT), leader, and engaged team member.

Current NP/PA Massachusetts License. Certification in respective specialty. Two years working as a licensed NP/PA in an ambulatory care setting; experience in a Community Health Center preferred. Fluent in Spanish and/or Portuguese, a plus. Competency in working with Microsoft Office Suite and Electronic Medical Records systems; familiarity with Epic is a plus. Reliable transportation and the ability to travel between sites as needed are required. Completion of NSCH compliance training. Required Skills and Abilities: Ability to work effectively both independently and as part of a team. Strong communication and interpersonal skills, committed to establishing a safe and respectful environment for team members. Proven ability to work in a fast-paced environment, with excellent time management skills. Ability to work in a culturally diverse environment. High commitment to continuous improvement in the quest for comprehensive, quality health care. Demonstrated interpersonal relationship skills. Work Environment Conditions Clinic, urgent care, mobile unit, telehealth, etc. Potential exposure to infectious diseases This position operates in a professional, fast-paced, and collaborative environment. Regular interaction with colleagues, supervisors, and/or clients occurs in person, by phone, and via digital communication platforms. Standard office hours apply, with occasional flexibility required to meet deadlines, attend meetings, or respond to organizational needs. Work may require sitting or standing for extended periods, using computers and other standard office equipment. Employees are expected to follow all organizational policies and procedures, including those related to safety, confidentiality, and ethical conduct.
Physical Requirements Frequent standing, lifting, and assisting with patient transfers Occasional lifting of materials (e.g., boxes, equipment) weighing up to 50 pounds Ability to sit or stand at a desk for extended periods, with consistent use of computers and telephones May be required to travel to multiple sites within a single day As a health center, NSCH expects all employees to receive all required vaccinations and health screenings yearly. Moreover, as a condition of employment, all employees are expected to be vaccinated against COVID-19, including providing proof of having received the COVID-19 vaccine, unless a religious or medical exception is granted.
