The Physical Therapist will conduct comprehensive evaluations and develop individualized treatment plans to be delivered in patients' homes. They will also supervise Physical Therapist Assistants and collaborate with an interdisciplinary team to ensure high-quality, compliant care.
Candidates must have successfully completed an accredited physical therapy program and hold a valid state license. A valid driver's license, auto insurance, and CPR certification are required, with home health experience preferred.
As a Home Health Physical Therapist, you’ll practice at the very top of your license, delivering skilled, one-on-one care directly in patients’ homes. No two days are the same, and every visit is an opportunity to create meaningful progress for someone in their most comfortable environment.
You’ll also lead and support Physical Therapist Assistants (PTAs), ensuring that care is delivered safely, effectively, and aligned with each patient’s goals. Working closely with an interdisciplinary team, you’ll continuously adjust treatment plans to drive the best possible outcomes.
