The Case Facilitator provides case management and daily living skills training to child and adolescent clients as part of a multidisciplinary team. They are responsible for linking clients to community resources, coordinating care, and ensuring the delivery of behavioral health services.
A Case Facilitator I - Child and Adolescent operates under supervision of the assigned Area Director or designated QBHP/LP. A member of a multidisciplinary treatment team, primary job duties include providing case management and activities of daily living skills to child and adolescent consumers. o Links, monitors, and refers clients to appropriate community resources o Coordinates care with appropriate internal and external parties o Ensures appropriate behavioral health services are provided
High School diploma or equivalent.
Must meet OBHP requirements. Two years’ experience in mental health or community services preferred.
Valid state issued drivers license.
Salary is based on experience.

Monday - Friday 40 hours a week. Flexible schedule with 2 evenings till 7pm preferred.
