The Community Health Worker will bridge patients to resources to address social determinants of health and provide health education. They will also offer informal coaching, counseling, and basic screening services as part of the client resource team.
Candidates must have a high school diploma or equivalent and a valid driver's license. Additionally, they must obtain CHW certification and BLS certification within the specified onboarding timeframe.
High school diploma or equivalent. Must obtain CHW certification through an approved curriculum within 12 months of hire date. Valid Driver's License required. This position may require a Motor Vehicle Driving Record Background Check to be completed during the onboarding new hire process and throughout employment. Must have a valid Class E license upon hire or Class E license must be obtained during orientation period. Current BLS certification upon hire or BLS certification must be obtained during orientation period or at the next available scheduled class.
