The Prevention Navigator assists clients with care coordination, including enrollment, appointment compliance, and access to essential services like housing and healthcare. They also conduct field-based outreach, perform rapid HIV/Hep C testing, and maintain accurate program data and documentation.
A high school diploma or GED is required, while a bachelor's degree in human services or public health is preferred. Candidates must have experience providing HIV health education and a strong understanding of issues affecting priority populations such as homelessness and substance use.
$21/hr - $23/hr
We have three comprehensive healthcare plans to choose from based on your priorities and budget. Housing Works covers most of the plan; you pay a portion, based on your salary. Staff begins accruing PTO immediately for a total of up to 30 days earned in the first year. We offer employees an educational benefit. This money is available for tuition loan reimbursement, tuition costs, and text books.
The Prevention Navigator will assist the Care Manager and Care Navigator with all aspects of care coordination for SAMHSA HOUSE 2.0 clients: complete initial enrollment (all required SAMHSA and program forms/assessments), promote compliance with appointments and treatment regimens, and encourage self-sufficiency. The Prevention Navigator will assist clients to access services, such as primary care, substance use treatment, mental health counseling, transportation, intensive care management, housing, and vocational/educational resources, through phone calls to clients and providers, and home visits when needed. The Prevention Navigator will participate in outreach events as needed as well as field-based client outreach.

This position is included in a bargaining unit of Housing Works’ New York City employees represented by a labor union known as the Retail Wholesale and Department Store Union (“RWDSU”). Accordingly, the RWDSU has the exclusive right to bargain over the terms and conditions of employment related to this position and this position may become covered by the terms of a collective bargaining agreement (a “CBA”) between Housing Works and the RWDSU
