The LPN will provide direct nursing care, assist with activities of daily living, and administer medications to residents. They are also responsible for monitoring resident health, updating service plans, and supervising nursing staff under the direction of a Registered Nurse.

Follow the service plan in providing services to residents and assist with their medical problems. Assist in the development and updating of each resident’s service plan. Assist with services needed to the residents (ADL's). Give residents prescribed medications; and/or remind them to take medications. Monitor health safety and well-being of the residents and communicate with the Registered Nurse and/or physician when needed. Document information pertaining to changes in residents' functional status, personal needs, appointments, etc. in the communications log and update the service plan and daily care log sheets if indicated. Assist in keeping the environment safe for residents. Provide one-on-one support/visitation to assigned residents on a weekly basis or as requested by the Registered Nurse or as specified in the assistance plan or daily communication log. Report concerns about residents to the Registered Nurse. Assist the Registered Nurse in any capacity he/she may need, including supervising nursing staff with training, work assignments and support as well as carry out all other duties assigned. Respond to resident emergencies, provide first-aid assistance and arrange for medical attention/follow-up. Maintain an accurate record of services given to residents. Adhere and carry out all policies and procedures. Report all job-related injuries to the Registered Nurse before the end of the shift.
Must be a current Licensed Practical Nurse by the state of NJ. Passed a training course approved by the NJS Department of Health and shall have passed the NJ Practical Nurse Examination.
Shift: Monday-Friday 7:00am-3:00pm every other weekend
