The Discharge Planning Coordinator is responsible for managing the transition of patients from the hospital to continued treatment settings. They develop and implement individualized discharge plans based on the specific needs of each patient.
A bachelor's degree in Psychology, Counseling, Social Work, or a related field is preferred. Candidates must also possess current CPR and First Aid certification, with at least one year of experience in psychiatric inpatient care preferred.
