The RN Case Manager develops and coordinates care plans for patients with terminal or chronic illnesses in collaboration with physicians and families. They provide ongoing clinical assessments and manage end-of-life symptom management and pain relief.
Candidates must be a licensed Registered Nurse in Vermont and New Hampshire or hold a compact license and be a graduate of an accredited nursing school. A valid driver's license and clean driving record are required, with hospice or oncology experience preferred.

Hospice RN Case Managers at the DHHC focus on providing pain management and comfort for patients with terminal or chronic illness, as well as communicate and coordinate care needs with the interdisciplinary team, the patient and their families/caregivers. The RN Case Manager: · Develops a care plan in collaboration with the patient's physician, the patient, and their family, or the primary caregivers upon admission. Periodically reviews the patient care plan and recertifies the patient care plan as appropriate. Coordinate the patient's care plan through interdisciplinary communication and case conferences, as appropriate. Provide ongoing assessment of the patient's clinical, functional, and emotional status to identify problems and/or progress toward desired goals. Communicate significant changes to the patient's physician and DHHC colleagues. Document evaluations, patient response, and progress towards goals. Teach the patient and caregiver care techniques and end-of-life symptom management as appropriate.
REQUIRED QUALIFICATIONS Must be a licensed Registered Nurse in Vermont and New Hampshire or hold a compact license. Graduate of an accredited school of nursing Medical/Surgical/Oncology nursing and/or hospice care experience preferred.
Minimum two years’ general experience in a supervised nursing setting recommended. Hospice/Homecare experience preferred. Certified in Hospice/Palliative Care Nursing desirable. Required Licensure/Certifications Unencumbered RN license in VT and NH or compact. Driver’s license, minimum requirements on car insurance, and a clean driving record
