The Community Resource Specialist acts as a liaison between members, service providers, and the community to facilitate access to physical, oral, and behavioral health services. They provide outreach, education, and advocacy to help members navigate the healthcare system and overcome barriers to care.
Candidates must have a high school diploma or GED, with a college degree preferred, and at least three to six months of related experience. A valid Oregon driver's license and the ability to obtain specific certifications like Traditional Health Worker (THW) within two years are required.

This position supports coordinated care members in accessing physical, oral, and behavioral health services by building trusting relationships and serving as a liaison between the member, service providers, and the community. Approximately 80% of engagement occurs via phone and email, with 20% involving in-person support. The role enhances service quality and cultural competence while increasing health literacy, knowledge, and self-sufficiency through outreach, education, informal counseling, social support, and advocacy.
This position may have voluntary on- call responsibilities.
This position does not have any supervisory responsibilities.
Requires the use of personal vehicle for local travel (subject to mileage reimbursement).

AllCare Health employs staff that are defined under OHA or CMS as health care providers. Positions that are defined as providers include, but are not limited to, Doctors, Nurse Practitioners, Physical Therapists, Traditional Health Workers, and Certified or Qualified Healthcare Interpreters.

All relevant experience can be listed in your resume to perform essential duties of the position including the following: lived, volunteer, professional, or a combination of experience and education.
High school diploma or general education degree (GED) required. College degree is preferred.
Ability to read and interpret documents such as safety rules, operating and maintenance instructions, and procedure manuals. Ability to write routine reports and correspondence. Ability to speak effectively before groups of customers or employees of organization. Verbal and written fluency in Spanish is preferred.
Ability to add, subtract, multiply, and divide in all units of measure, using whole numbers, common fractions, and decimals. Ability to compute rate, ratio, and percent and to draw and interpret bar graphs.
Has advanced basic computer job skills including logging on to systems, ability to communicate by email, ability to compose documents, enter database information, create presentations, download forms, and preserve/backup important data.
Ability to apply common sense understanding to carry out instructions furnished in written, oral, or diagram form. Ability to deal with problems involving several concrete variables in standardized situations.
