The Nursing Coordinator will triage participants, administer psychotropic medications, and coordinate medical care within a mental health team. They will also act as a liaison between pharmacies and insurance providers while documenting all participant interactions.
Candidates must hold an LVN certification from an accredited nursing school and a current California license. A valid California driver's license, personal vehicle, and proof of insurance are also required.
BACS is seeking an LVN for our nursing coordinator position in Sacramento working with homeless individuals in the community. The Nursing Coordinator is part of a mental and health care team providing medical case management coordination and care to participants.
This is a field-based position and must be able to work in the community.
The THRIVE programs are new initiatives by Sacramento County Behavioral Health to provide services specifically for adults who are justice involved and meet criteria for specialty mental health services. Justice involved may include participating in mental health diversion, mental health court, parole/probation, etc. THRIVE Outpatient Program will serve the moderate level community members and be open a standard Monday-Friday. THRIVE FSP will serve the high intensity community members and operate seven days a week.
Monday-Friday Day Shift.
