Requires a degree in Social Work or a related field like Psychology, with a Master's degree preferred. Candidates must have at least one year of social work experience, preferably in a hospice setting.
Trustbridge Hospice, a member of Empath Health, is seeking a compassionate and patient-focused Clinical Social Worker to join our team.
30 a.m. – 5 p.m, and 1 weekend every six weeks. In this important role, you will provide essential psychosocial assessments and interventions that help patients and families navigate end-of-life care—directly enhancing the quality, dignity, and comfort of their hospice experience. The social worker visits our hospice patients in the home, ALF, or SNFs in Belle Glade, Loxahatchee, West Palm Beach, Wellington.
