The LPN Coordinator is responsible for dispatching staff and coordinating patient care needs in collaboration with managers and physicians. They serve as a professional leader in assessing and guiding patient care activities to ensure high-quality, compassionate service.
Candidates must hold a current Florida Licensed Practical Nurse license and a Basic Life Support certification. A minimum of one year of experience in a relevant nursing field such as hospice, home health, or geriatric care is required.
Trustbridge Hospice, a part of Empath Health is seeking a caring and motivated LPN Coordinator/Dispatcher to work with us in our West Palm Beach location to provide coordination and dispatch of our staff visits on the following schedule: Monday - Friday 8:00am – 4:30pm.
