Requires a Bachelor's or Master's degree in Occupational Therapy and an Indiana OT license. Candidates must also possess CPR certification and have the ability to travel up to 50% of the time.

WHAT YOU CAN EXPECT Practice Hours: Monday – Friday, 8:00 a.m. – 5:00 p.m. No Weekends, Evenings, or Holidays Performs comprehensive occupational therapy evaluations, treatment and related follow-up services including discharge planning. Develops a treatment plan with measurable goals based on patient assessment. Completes appropriate goal re-assessment and discharge planning. Provides occupational therapy, including selection and use of assistive equipment. Trains and educate patients and family members or other support and document understanding of treatment plan and goals. Documents and submit patient reports and charges within the established timeframe. Collaborates with patient and other healthcare personnel or outside agencies regarding coordination of care. Supervises Occupational Therapy Assistants and Rehab Techs QUALIFICATIONS Bachelor's Degree or Master's Degree- Occupational Therapy - Required Doctorate- Occupational Therapy- Preferred Occupational Therapist (OT) License- State Licensing Board- Indiana- Required within 90 Days Cardiopulmonary Resuscitation (CPR) American Heart Association- Required 1 Year Occupational Therapy Experience- Preferred TRAVEL IS REQUIRED: Up to 50% EQUAL OPPORTUNITY EMPLOYER It is the policy of Franciscan Alliance to provide equal employment to its employees and qualified applicants for employment as otherwise required by an applicable local, state or Federal law.
