The Case Manager will provide direct services to clients, including intake, assessment, and service plan development while managing a caseload. They are also responsible for conducting outreach, home visits, and coordinating client services with various community providers.
Candidates must hold a Bachelor's degree and possess relevant case management experience. Proficiency in Microsoft Office and the ability to perform physical tasks like climbing stairs for home visits are required.
