About the role

Provide direct social services including intake, assessment, and therapeutic counseling for men experiencing homelessness with mental illness. Manage a caseload by coordinating with medical and social service providers to facilitate placement into permanent housing.

Across our 60+ locations, we provide programs in a variety of service areas, including workforce development, children, youth, families, seniors and homeless prevention.

The Jerome Avenue Men’s Shelter (JAMS) provides beds to 200 men with mental illness who are experiencing homelessness. The site offers an array of social services, such as housing case management (including assistance with benefits and gathering vital documents), programming to address mental health and substance abuse issues, meals, and activities. The site has an on-site clinic that can provide primary care and psychiatric services. Residents can remain at the shelter until placed into permanent housing, ideally within nine months of receiving services.

Qualifications

  • New york state lmsw or lcsw required (Or obtained within 6 months of hire)
  • Proficiency in English required; proficiency in Spanish preferred
  • Proficiency in modern business communications including personal computers, electronic mail, voicemail, facsimile and copier equipment
  • Familiarity with AWARDS and CARES (or similar databases) preferred.
  • Driver’s license preferred, but not required.

Essential

Duties and Responsibilities

  • Provide direct services to clients, including engagement, intake, assessment (including psychosocial assessment), service plan development, referral, advocacy, therapeutic counseling, reassessment, and exit summaries.
  • Manage a case load.
  • Monitor and document client progress toward service plan goals; document all interactions with, or on behalf of, clients.
  • Provide group counseling as assigned by the assistant program director or other supervisor.
  • Maintain familiarity with program resources available on- and off-site.
  • Coordinate client services with mental health, medical, employment, educational, child care and other social service providers.
  • Conduct home and field visits as required.
  • Conduct regular case conferences with clients and service providers as required.
  • Assist with special projects as assigned including representing agency at interagency events (as assigned).
  • Act as a program contact person for inquiries concerning clients (as assigned).
  • Supervise all office functions in the supervisor’s absence (as assigned).
  • Report to and meet with the supervisor on a regular basis.
  • Complete (or assist with completion of) program reports.
  • Perform additional duties as assigned

Program Specific Responsibilities

  • Complete HRA housing applications for all clients on caseload.
  • Document client interactions and complete other service-related documentation in program databases (including AWARDS and CARES) in a manner consistent with DHS and/or internal procedures.
  • Work additional hours and shift as needed (evening and/or weekend hours may be required).
  • Perform on-call duties (as assigned).
  • Obtain 9.58 credential to conduct involuntary mental health removals.
  • Assist with client locker searches as requested.

Skills/Abilities

  • Ability to use a computer for prolonged periods
  • Ability to occasionally lift and/or move up to 10 pounds
  • Ability to stand, walk, or sit for long periods of time
  • Ability to climb five flights of stairs
  • Ability to bend and retrieve objects and/or documents
  • Ability to travel in the boroughs of New York City and its adjacent counties via public transportation
  • Ability to travel for a minimum of 1 mile within a reasonable amount of time and in all types of weather, including inclement weather

Requirements

Requires a New York State LMSW or LCSW license (or obtainment within 6 months) and proficiency in English. Candidates must be able to perform physical tasks such as climbing stairs and traveling within New York City boroughs.
