Responsibilities
- Independently manage a caseload
- Conduct comprehensive intake assessments.
- Conduct regular field visits to engage and build rapport with chronically homeless individuals.
- Work with clients to develop goals and service plans.
- Provide referrals and direct assistance to support clients applying and/or attending appointments
- for mental health services, medical care, substance treatment, vital documents, benefits,
- legal/immigration services, and permanent housing.
- Document all interactions with or on behalf of clients, monitor and document client progress
- toward service plan goals.
- Maintain familiarity with social services and resources available both on-site and in the
- community.
- Coordinate client care with other service providers; attend regular case conferences with
- clients and other service providers, as required.
- Attend regular supervision sessions with direct supervisor.
- Drive agency vehicle to and from client locations and appointments
- Perform additional duties as assigned.
Qualifications
- Bachelor’s degree in social work or a related field of study
- Candidate must have a valid driver’s license which has been held for at least 1 year; in the last 3
- years, candidate must have obtained no more than 3 license violation points
- One-year of case management or related work experience preferred.
- Ability to engage and build rapport with populations with high support needs.
- Strong oral and written communication skills
- Detail-oriented and resourceful with strong time management and organizational skills
- Proficiency in Microsoft Office suite and other standard business technology
- Bilingual/multilingual preferred, but not required.
- Valid driver’s license is required for this position.
- Physical requirements
- Ability to use a computer for prolonged periods.
- Ability to occasionally lift and/or move up to 10 pounds.
- Ability to stand, walk, or sit for long periods of time.
- Ability to bend and retrieve objects and/or documents.
- Ability to travel in the boroughs of New York City and its adjacent counties via public
- transportation.
- Ability to travel for a minimum of 1 mile within a reasonable amount of time and in all types of
- weather, including inclement weather, if required to conduct home visits and/or field work
- TB Test required within the first 120 days of employment.
