The Social Worker provides direct clinical services, including assessments, treatment planning, and counseling for a diverse client population. They act as a liaison between clients and community-based providers to coordinate housing, mental health, and social services.

Responsibilities

  • Interaction with clients, community-based service providers
  • Act as a liaison with clients and community-based service providers.
  • Provide direct services to clients, including Bio-psycho-social assessment, engagement, treatment plan development, community-based service referrals, advocacy, counseling, exit summaries and follow-up
  • Escort clients to Supportive Housing interviews.
  • Outreach and establish relationships with SH vendors/organizations.
  • Outreach and establish relationships with Mental Health service centers/providers.
  • Maintain familiarity with program resources available on- and off-site.
  • Coordinate client services with mental health, medical, employment, educational, childcare and other social service providers.
  • Attend & participate in shelter meetings (i.e. Interdisciplinary, School Attendance, Group Supervision, etc.) .
  • Provide direct services to clients, including engagement, intake, assessment, service plan development, referral, advocacy, therapeutic counseling, reassessment, and exit summaries.
  • Documentation
  • Monitor and document client progress toward service plan goals; document all interactions with, or on behalf of, clients.
  • Complete program reports.
  • Review and sign off on service plans and psycho-social assessments as assigned.
  • Accurately and timely complete and compile 2010e (supportive housing applications).
  • Complete treatment plans as required.
  • Provide daily Bio-psycho-social assessment updates to Director of Social services and Residence Director.
  • Monitor and document client progress toward housing service plan goals/exit strategy.
  • Administrative
  • Complete biopsychosocial within 30 days of admission.
  • Manage a caseload for a diverse client population.
  • Provide group counseling as assigned by the supervisor.
  • Conduct regular home and field visits as required.
  • Conduct regular case conferences with clients and service providers as required.
  • Assist with special projects as assigned including representing agency at inter-agency events.
  • Act as program contact person for inquiries concerning clients.
  • Co-facilitate regular case conferences with clients and service providers as required.
  • Trainings/Supervision
  • Attend DHS CCC trainings/meetings as required
  • Attending individual and group supervision with the clinical supervisor as scheduled.
  • Report to, and meet with, supervisor on a minimum monthly basis.
  • Attend strategic planning supportive housing meetings as required.
  • Perform other duties as assigned by the manager.

Qualifications

  • A master's degree in social work is required.
  • New York State LMSW certification required, or the ability to obtain license within six (6) months of hire.
  • Proficiency in English required; proficiency in a second language preferred.
  • Strong oral and written communication, time management, and organizational skills are necessary.
  • Proficiency in Microsoft Office suite and other standard business technology is required.
  • Skills/Abilities
  • Ability to use a computer for prolonged periods
  • Ability to occasionally lift and/or move up to 10 pounds
  • Ability to stand, walk, or sit for long periods of time
  • Ability to climb five flights of stairs, if required, to conduct home visits and/or fieldwork
  • Ability to bend and retrieve objects and/or documents
  • Ability to travel in the boroughs of New York City and its adjacent counties via public transportation
  • Ability to travel for a minimum of 1 mile within a reasonable amount of time and in all types of weather, including inclement weather, if required to conduct home visits and/or field work

Requirements

A Master's degree in Social Work and an LMSW certification (or the ability to obtain one within six months) are required. Candidates must be proficient in English and Microsoft Office, with the physical ability to conduct field visits across New York City.
