The Nurse Practitioner will perform comprehensive health assessments, diagnose and manage acute and chronic conditions, and develop individualized care plans for residents in skilled nursing facilities. They will also collaborate with an interdisciplinary team to ensure coordinated care, reduce hospitalizations, and improve patient outcomes.
Candidates must hold a Master's degree in Nursing from an accredited program and possess an active, unrestricted Advanced Practice Nurse license. A minimum of 2 years of clinical experience, preferably in geriatrics or long-term care, is required along with current DEA registration.
Vitae Health is seeking a dedicated Nurse Practitioner to provide high-quality, patient-centered medical care to residents in a skilled nursing facility. The NP will assess, diagnose, and manage acute and chronic conditions while collaborating with an interdisciplinary care team to improve patient outcomes and quality of life.
