Responsibilities

• Plan and conduct segments of treatment programs designed to restore and improve physical function, with attention to social and mental aspects, in a home health setting.
• Serve as a co-leader in an interdisciplinary approach to patient groups, coordinating with team members to achieve care objectives.
• Assess patient progress, adjust therapy plans as needed, and document outcomes in patient records.
• Educate patients and caregivers on exercises, safety, and activities of daily living to promote independence.
• Collaborate with nursing, therapy assistants, and other clinicians to ensure continuity of care and appropriate discharge planning.
• Ensure therapy is delivered in alignment with the plan of care, safety standards, and regulatory requirements.

Requirements

• Current physical therapist license in California (CA)
• Ability to practice in a home health care environment
• Eligibility for travel assignment
• Strong communication and documentation skills
• Ability to work with an interdisciplinary team and adapt to patient-centered care
