Responsibilities
Provide direct care for adult med/surg telemetry patients, including ongoing assessments and telemetry monitoring; respond quickly to changes in patient condition and escalate as needed; document in EPIC.
Administer prescribed medications and maintain IV access; perform phlebotomy and related procedures as required; collaborate with the care team to ensure accurate charting and reporting.
Manage devices and basic procedures: Foley catheter care, NG/G-tube care, oxygen delivery, wound care and dressing changes; assist with feeding and nutritional needs as appropriate.
Perform routine EKG monitoring (12-lead), monitor blood glucose, and participate in isolation precautions and infection control practices; communicate findings via SBAR for transfers and handoffs.
Coordinate care with interdisciplinary teams (IV team, PT, Respiratory, Radiology, Pharmacy, Social Services, Transport, Unit Secretary, Rapid Response) and participate in unit turnover and handoffs; float to med/surg as needed.
LPN scope note: may not assume assignment or act as a resource in GI/Med-Surg per unit policy; comply with unit guidelines while delivering patient-centered care.
